Important Notice to Our Patients

Dear Patients,

After careful consideration, I am writing to let you know that, due to personal reasons, I have made the difficult decision to close Coupet Family Medicine. The practice will close at the end of the day on November 4, 2026, and I will no longer provide patient care through Coupet Family Medicine after that date.

This was not an easy decision. I am saddened that I will not be able to continue serving as your physician. It has been a privilege to care for you and your families, and I am deeply grateful for the trust that you have placed in me over the years.

Between now and November 4, I will do everything reasonably possible to help make your transition to a new primary care physician as smooth as possible.

Because the practice will be closing, please begin searching for a new primary care physician now rather than waiting until the end of the year. I will be providing information and recommendations for other physicians and practices in the area shortly. You may also wish to contact your health insurance company for a list of participating primary care physicians accepting new patients.

I will make reasonable efforts to assist with your ongoing medical needs. Appointment availability will be very limited, so patients with routine or ongoing medical needs are encouraged to arrange care with a new physician promptly. 

Patients are encouraged to begin establishing care with a new primary care physician as soon as possible. You may contact your health insurance company for assistance locating an in-network physician who is accepting new patients.

Appointment availability at Coupet Family Medicine will be limited during the remainder of October.

Please review your current medications and contact our office promptly regarding medications that may require refills during your transition to a new physician.

Refills will remain subject to appropriate medical evaluation and clinical judgment.

Your medical records will remain available after the practice closes.

Patients may request that their records be transferred to a new physician or obtain a copy for their own records. 

Please complete Release of Medical Information Form below to request your records and send the completed form to patient@coupetfm.com.

We are unable to email medical records because it is not secure enough to protect your personal health information. 

Please contact our office if you need any assistance with medical record requests.

Someone from our office will remain available to answer telephone calls and messages during regular business days from 9:00 AM to 5:00 PM. 

Because we anticipate increased call volume during this transition, response times may be longer than usual. Please leave one detailed message rather than making multiple calls regarding the same request.
